APCM: Enhancing Care and Outcomes in FQHCs
APCM incorporates elements of Chronic Care Management, Transitional Care Management, and Principal Care Management. Our care managers will reach out to your patients via phone or text on a regular basis to help them set up appointments, manage their prescriptions, transition between care settings, and find Social Determinants of Health (SDOH) resources, such as transportation agencies, housing, utilities, and nutrition support. By streamlining care and addressing social needs, APCM empowers FQHCs to improve patient outcomes and enhance health equity.
Medicare's APCM model: features three levels:
Level 1
For patients with one or no chronic conditions
($15 reimbursement per patient, per month)
Level 2
For patients with two or more chronic conditions
($50 reimbursement per patient, per month)
Level 3
For patients with two or more chronic conditions who are Qualified Medicare Beneficiaries (QMB)
($110 reimbursement per patient, per month)*
