Advanced Primary Care Management (APCM) is designed to support the vital work of primary care providers. Our APCM care managers act as an extension of your team, proactively engaging with your Medicare patients between appointments to create health goals, assist with medication management, connect them with resources for social needs, and follow up after hospital or ER visits. Improve patient outcomes and expand the care your practice can provide.
Medicare's APCM model: features three levels:
Level 1
For patients with one or no chronic conditions
($15 reimbursement per patient, per month)
Level 2
For patients with two or more chronic conditions
($50 reimbursement per patient, per month)
Level 3
For patients with two or more chronic conditions who are Qualified Medicare Beneficiaries (QMB)
($110 reimbursement per patient, per month)*
