ChartSpan Announces Partnership with Remote Care Partners

(Greenville, South Carolina) - January 13, 2021 - ChartSpan, the largest provider of fully-managed Chronic Care Management (CCM) services in the U.S, announced a new partnership with Remote Care Partners (RCP), a cloud-based Remote Patient Monitoring (RPM) platform.
